Usually what people do is go to a USCIS-designated "civil surgeon". The
doctor checks your vaccination history, gives you any needed vaccinations,
and fills out a vaccination supplement. Then they put it in a sealed
envelope for you to include with your AOS application. Sometimes the
doctors are confused and try to give you a complete medical exam. When you
call you make your appointment, make sure they understand that you just need
the vaccination supplement.

You can find a list of civil surgeons on www.uscis.gov.

There is no form designated I-693A. That designation is the invention
of DIY newsgroupers who like to abbreviate and use acronyms so that
they don't have to type so much. The form is actually Attachment A
Supplemental Form to I-693. Google

Not knowing what vaccinations your fiancee has already had and has
documented on her DS-3025, and not being a medical doctor who has seen
your fiancee and has reviewed her medical history, it's hard to say if
she will need to get additional vaccinations.

My wife just submitted her DS-3025 for her adjustment of status, no
I-693 vaccination supplement, because her DS-3025 documented that
she'd had all of the vaccinations medicaly appropriate for her that
are required for immigration.
