From your description, the crossed line was almost certainly due to
misoperation of the switchboard at the call centre, and so nothing to
do with BT. It used to be against BT rules to have a switchboard in
the UK that could connect two outside callers together, but the US
switchboards have had no such restriction AFAIAA.

The new PABX we have had fitted at the office is able to connect two
outside calls together, so I guess that restriction has now been
lifted. Handy if you want to put an outside call through to an
employee who is off-site without either letting the caller know the
person is not in the office (or maybe even the country), or give out
private mobile or home numbers.
