Usually you travel with the valid visa in the old passport along
with the new passport and present both for admission to the US.
They may punch holes or otherwise void the old passport, but leave
the visa intact.
Some consulates have been known to transplant visas into the new
passport (Italy is one) but this is illegal and can cause BIG problems
at the border.
Removing a visa from the passport it was placed in makes it void
pursuant to 22 CFR 41.122(h)(8). Applying to enter the US without a
valid visa can lead to a 5 year removal order.
I'm not sure how the UK does it.
